CE Phlebotomy Program Requirements
Photocopy of a valid Driver’s license or state issued ID.
Applicant must provide a valid email address, as this will be how applicant will be contacted.
High School diploma or GED.
* If accepted; A clear and approved background check must be on record before registration is complete.
One current TB test with negative results (TB skin test is only valid for one year, and it must be valid through the duration of the course end date); or chest x-ray report within the last 5 years clearly indicating no tuberculosis disease is present.
Hepatitis B – A 6 months series or serologic confirmation of immunity (titer).
MMR – 2 doses or serologic confirmation of immunity (titer).
Varicella – 2 doses or serologic confirmation of immunity (titer).
Current Tdap (within the last 10 years).
The Phlebotomy Program is 13 weeks of lecture/lab, plus 3 weeks of clinical rotation. (The clinical rotation will be determined by available sites for students). The classes will be held Monday, Wednesday, and Friday, 5:30p to 8:30p. Classes will be held in Edna.
The total cost for the program is $850.00. There will be a $283.00 registration fee (this will be to reserve a spot and pay for the workbook and is non-refundable), The student can choose to pay tuition in 3 installments of $283.00, first installment due at registration and is non-refundable. If a student chooses to drop the course before 4 weeks, they will be refunded the tuition, minus the $283.00 registration fee(non-refundable). After 6 weeks, the tuition will not be refunded.
The student will also need to carry professional liability insurance for their clinical rotation. I suggest looking at Proliability for coverage options available.

If the student chooses to pay tuition in payments, the student must complete the payment option agreement and agree to the terms outlined.
